A coroner’s Prevention of Future Deaths report has listed 14 concerns about care at Newham Mental Health Centre after Hugo Flint Cahan was strangled by another patient in January 2023. East London NHS Foundation Trust and NHS England must reply by 19 November, while one staff member has been dismissed and four are under investigation.

Two men from Luton have been sentenced for creating over 2,000 fraudulent Covid-19 vaccination records between June and October 2021. Waqas Hanif, who worked at Kingsway Health Centre, was jailed for three years, while Touqir Nasir received a suspended sentence. The scheme earned them tens of thousands of pounds.
